AI Form Generation
Use AI to generate a complete form from a plain-English description, then refine it in the builder.
Open in appAI form generation is the fastest way to create a form. Instead of dragging fields one by one, you describe what you need in plain English and BizForms builds the complete structure — field types, labels, validation rules, and section breaks — in seconds.
How to use it
- From the dashboard, click New Form
- Select Describe it (AI)
- Type a description of your form in the text box
- Click Generate
The AI will create a form draft and open it in the builder. From there you can edit, reorder, add, or delete fields as normal.
Writing a good prompt
The more specific your description, the better the output. Include:
- Purpose — what the form is for
- Audience — who fills it in (internal staff, customers, patients)
- Fields — specific information you need to collect
- Validation — any required fields or format constraints
- Structure — whether you want sections or a single-page form
Examples
Basic:
"A contact form with name, email, phone, and message. Name and email are required."
Detailed:
"A job application form for a software engineer role. Section 1: personal details (name, email, phone, LinkedIn URL). Section 2: work history (current employer, job title, years of experience as a number). Section 3: a file upload for CV (PDF only, max 5MB), and a long text field asking why they want the role. All fields required except LinkedIn."
Healthcare:
"A HIPAA patient intake form. Fields: legal name, date of birth, gender (dropdown), insurance provider, policy number, primary care physician name, current medications (long text), allergies (long text), emergency contact name and phone. Mark all health-related fields as PHI."
Editing the AI output
Treat the AI output as a first draft. You should always review and adjust:
- Field labels — make them match your brand's voice
- Validation rules — check min/max values, required toggles
- Field order — drag to reorder as needed
- Section structure — add or remove section breaks
Re-generating
If the output isn't right, you can click Re-generate from the AI panel, optionally updating your prompt first. You can also use the Add fields via AI button in the builder to add more fields to an existing form without replacing the whole structure.
Plan requirements
AI form generation is available on Pro and Business plans. Free and Basic users can build forms manually.
